This can be caused by one of three areas:

Your mobile phone split the Questionnaire link into two different SMS messages.
Please look at your phone settings for combining messages.

The Questionnaire does not match the current date.
Please access the most current Questionnaire.

You are already logged into an ActionTrac admin panel.
Please log out of your reports or admin panel and then click on the Link again.
